Abstract
Background : Discharge planning is the provision of information to patients and families to know how to manage nutrients after being treated in Hospital, and know what the risk factors that can make the disease relapse and treatment at home later against the disease. Discharge planning aims to minimize the impact of a health condition such disease with continuous treatment. Purpose : This study was to fully assess the effect of discharge planning on nutrition knowledge management of patients with type II diabetes mellitus. Research method : This study made used pre experiment with one group preposttest design. The sampling in this research are 15 patients with type II diabetes mellitus in PKU Muhammadiyah Hospital of Yogyakarta with kuota sampling. Analysis of the research data used independent t-test. Result : The analysis of independent t-test obtained there is a difference on nutrition knowledge management of patients with type II diabetes mellitus before and after gave discharge planning (p<0,05). Conclusion : Granted discharge planning affect increased nutrition knowledge management of type II diabetes mellitus. Suggestion : It is expected that patients using the information provided is to be understood about the nutrition management of his own after being treated in Hospital.
